Investigation Underway Following Suicide at Audie L. Murphy Memorial Veterans Hospital

The San Antonio Police Department (SAPD) is conducting an investigation into the death of a patient by suicide at the Audie L. Murphy Memorial Veterans Hospital, which occurred on April 7, 2025. The incident has raised concerns about the adequacy of mental health support provided to veterans at the facility.
The Audie L. Murphy Memorial Veterans Hospital, located at 7400 Merton Minter Boulevard in San Antonio, is a key component of the South Texas Veterans Health Care System. It offers a comprehensive range of services, including mental health care, to veterans in the region.
This recent tragedy is not an isolated incident. A report by the Department of Veterans Affairs Office of Inspector General (OIG) highlighted deficiencies in the management of a previous suicide case at the same facility. The report identified issues such as inadequate assessment of suicidal behavior, failure to establish effective safety plans, and insufficient documentation.
In response to these events, the hospital administration has reiterated its commitment to improving mental health services. Efforts are underway to enhance staff training, implement more robust safety protocols, and ensure better coordination with crisis intervention services.
